Abstract:
اهداف: بیماران قلبی سالمند دیابتی قشری آسیبپذیر هستند، نظر به اهمیت موضوع ، مطالعه حاضر با هدف مقایسه شیوع عوامل خطر، نوع درمان ریپرفیوژن و مرگ و میر بیماران سکته قلبی سالمند در دو گروه دیابتی و غیردیابتی انجام شد.
مواد و روشها: مطالعه حاضر از نوع کوهورت است و جامعه آماری آن تعداد 1460 بیمار سالمند که در قالب طرح ریجستری بیماران STEMI بیمارستان امام علی (ع) کرمانشاه از تاریخ 95/4 تا تاریخ 98/9 بستری شده بودند را شامل میشد. پس از تکمیل فرم رضایت آگاهانه و بر اساس معیارهای ورود، به دو گروه دیابتی و غیر دیابتی تقسیم شدند. اطلاعات بیماران در قالب چک لیستی شامل وضعیت دموگرافیک، سوابق پزشکی، ریسک فاکتورهای بیماری قلبی-عروقی، معاینات بالینی، آزمایشهای تشخیصی، درمان و مرگ در هنگام پذیرش بیمار، در زمان بستری و هنگام ترخیص توسط پرستاران آموزش دیده تکمیل شد. تحلیل دادهها با استفاده از آمار توصیفی، T مستقل، من ویتنی و آزمون کای دو در نرم افزار Stata نسخه14انجام شد و 0/05>P به عنوان سطح معناداری در نظر گرفته شد.
یافته ها: نتایج مطالعه حاضر نشان داد تعداد زنان دیابتی(47/11درصد) بصورت معنی داری(0/001>p) بیشتر از تعداد زنان در گروه غیر دیابتی(25/60درصد) است، همچنین شیوع مقادیر میانگین LDL و کلسترول در غیر دیابتیها بطور معنی داری(0/001>p) بیشتر از دیابتی ها بود. شیوع میانگین HDL نیز در دو گروه با یکدیگر اختلاف معنی داری نداشت (0/777>p). شیوع متغیرهای BMI≥30، سابقه سکته قلبی، چربی خون، فشار خون، تریگلیسرید و GFR در گروه دیابتیها بصورت معنی دار بیشتر از غیر دیابتیها بود. در مورد متغیرهای محل زندگی، سطح سواد، مصرف سیگار و دخانیات، سوابق سکته مغزی، آنژیوپلاستی عروق کرونر و جراحی پیوند عروق کرونر در دو گروه با یکدیگر اختلاف معنا داری نداشتند. همچنین در نرخ مرگ و میر و درمان ریپرفیوژن اختلاف معنی داری در دو گروه مشاهده نشد.
نتیجه گیری: شیوع برخی عوامل خطر ناشی از سکته قلبی در سالمندان دیابتی و غیر دیابتی متفاوت است و نتایج این مطالعه با فراهم کردن الگویی علمی مبتنی بر دادههای ریجستری اطلاعات سودمندی را به متخصصان این حوزه جهت تدوین برنامههای اصلاح سبک زندگی با هدفمندتر کردن تجویز دارویی و آگاهی بخشی به صورت خاص به این قشر از بیماران ارائه مینماید.
Objectives: Elder Myocardial infarction diabetic patients are vulnerable, considering the importance of this issue, our aim in this study was to compare the prevalence of risk factors, type of reperfusion therapy and mortality of elderly MI patients in diabetic and non-diabetic groups.
Methods: The present study is a cohort and its participants included 1460 elderly patients who were admitted to Kermanshah Imam Ali (AS) Hospital from 2016 to 2019 as part of the STEMI patient registry program. After completing the informed consent form and based on the inclusion criteria, divided into diabetic and non-diabetic groups. Patients' information was completed in the checklist including demographic status, medical history, cardiovascular risk factors, clinical and diagnostic tests, treatment and mortality at the time of admission, hospitalization and at the time of discharge by trained nurses.
